Having insurance doesn't guarantee cheap prescriptions. Here's why your medication might still cost a lot — and what to do about it.

Reason 1: You Haven't Met Your Deductible

Many plans require you to meet your deductible before drug coverage kicks in. Until then, you're paying the negotiated rate — which is still cheaper than retail, but can still feel high.

Reason 2: It's a High-Tier Drug

Brand-name and specialty drugs sit in higher formulary tiers with higher cost-sharing. Your plan might cover 60% of a Tier 3 drug — but 60% of $400 is still $160 out of pocket.

Reason 3: Prior Authorization is Required

Some drugs — especially expensive ones — require your doctor to get pre-approval from the insurer before they'll cover it. Without that approval, you pay full price. This is called prior authorization (PA).

Reason 4: Specialty Pharmacy Routing

Some plans require specialty drugs to be filled through a specific specialty pharmacy. Using your regular pharmacy means paying out of pocket.
